I further authorize all corporations, companies, organizations, former employers and educational institutions to release to SME any and all information they may have about me that may verify the information provided on this application and release all such parties from any liability for doing so.

I understand the requirements of the office. I will run for election if selected by the Nominating Committee and will serve if elected. If elected, I will resign elected officer positions as specified in the SME Governance Document so as not to serve dual elected offices.

If elected to the SME Board of Directors, I agree to become a member of SME (if not currently a member) and maintain my membership through my term office.
